Healthcare Highways Announces Leadership Transition to Drive Revenue Growth
Care Coordination is a personalized service that helps you navigate the complexities of the healthcare system and provides you with the support you need on your journey to health and wellbeing. Care Coordinators (sometimes called Care Managers or Case Managers) are healthcare professionals who work directly with you and your providers to identify, understand, and take control of health risks and chronic conditions to ensure you receive appropriate and timely care.
Most importantly, as a member of Healthcare Highways Health Plan, care coordination services are provided at no cost to you and qualified, covered family members. That’s right – it’s already covered.
Care Coordination is for all members of Healthcare Highways Health Plan who may benefit from additional support managing their healthcare needs. Some examples include individuals who have chronic or complex medical conditions and individuals who will/have experienced major medical episodes, like large surgeries and childbirth. This also includes individuals who are at high risk of developing chronic conditions.
If you think you may benefit from Care Coordination, reach out to your dedicated Healthcare Highways Health Plan customer experience team and let the Customer Experience Advocate you’re speaking to know that you’re interested in enrolling in the Care Coordination program.
